The SEND ASSESSMENT PACKAGE

What is this?

This is a comprehensive, three-part, support package. It is designed to help schools and parents understand a learner’s needs and put the right provision in place quickly.

What does it include?

Learner Discovery Review

A structured, strengths-based exploration including:

Standardised/screening assessments (language, reading, spelling, numeracy, processing indicators)
Classroom and unstructured observational assessments
Learner voice
Parent voice
Review of current provision and progress

This gives a clear picture of the learner’s starting point.

Learner Evaluation Report

A detailed written report including:

The results of assessment
Description of strengths and barriers
Suggested outcomes and next steps
Suggested reasonable adjustments
Recommended strategies and interventions
Guidance on pathways to external services
What evidence may be needed for future referrals/EHCP requests

The report can be used directly in provision maps, meetings, and applications.

Planning Meeting

Once the report is completed, we meet with you to:

Discuss the findings
Agree realistic, measurable next steps
Create a short-term action plan
Support staff to implement recommendations
Identify where further evidence or screening may be needed
Plan follow-up dates and review points (if the service is requested again)

Please be aware that these assessments provide indicators of need but do not constitute a formal diagnosis.

Following the completion of this package, schools can commission additional services, including EHCP and referral support.
